Fee Schedule

My goal is to provide high-quality care that puts patients first.

I am in network with the following insurance companies:

​

• United Healthcare, Oscar, Oxford (including Optum, UMR, Golden Rule, Optum Behavioral Health, US Health group, and GEHA)

• Aetna (including Meritain, Coventry, Nippon Life Benefits, Chapman Boone, and GEHA)

• Cigna (including Evernorth)

If you do not have insurance coverage or are covered by insurance that I am not currently in-network with the self-pay rates below will apply:

Fees:

• Initial Evaluation (up to 90 minutes): $275

• Med Management follow-up (30 minutes): $150

• Extended Med Management (60 minutes): $225

I do not charge for clinically relevant phone calls (such as dosage adjustments during a titration or taper, questions about possible side effects, reviewing lab results, etc.), refill requests, or to fill out forms such as prior authorizations.

I will not ask you to pay for more than one appointment per month. If you need this level of care, we will discuss options (such as finding a therapist for additional support or finding a prescriber that takes your insurance or offers sliding-scale fees).  While there are some individuals that may benefit from monthly appointments, many people find that once symptoms have stabilized, following up every 2-3 months meets their needs.

I can provide you with a superbill after each visit that can be submitted to your insurance company for possible reimbursement. Reimbursement rates will vary for out-of-network services depending on your specific plan, but may help defray the cost of treatment.
